Job description
Main duties and responsibilities
Ensure that care is delivered safely and appropriately
Main activities:
  • Provide care according to all relevant health & safety guidelines, policies, procedures and regulations and in line with training
  • Attend all calls on the rota at the pre-arranged times
  • Keep in daily contact with the office as per the ILA Staff Safety procedure
Provide individualised home care
Main activities:
·         Support customers to live independently
  • Provide personal care to customers in their own home as identified in the Customer Plan
  • Provide domestic assistance as identified in the Customer Plan, which could include cleaning, food preparation, laundry, shopping, accompanying the customer in the community
  • Transport customers to appointments and activities
  • Assist with exercises, and prescribed plans and follow any medical instructions which apply carefully and consistently
  • Organise time and resources based on the individual needs of customers
  • Manage any unanticipated events or unstable situations
  • Keep accurate and up-to-date notes relating to tasks undertaken in a customer’s home
  • Inform the office of any changes to a customer’s situation or needs which need to be addressed
  • Report immediately to the office any illness, accident or non-entry to a customer’s home
  • Monitor supplies and resources in a customer’s home
Assist in co-ordination of community resources and supports
Main activities:
  • Encourage customers and families to be involved in care, if appropriate
  • Liaise with all family, medical and other resources, as required
  • Assist the customer in arranging for additional services and resources, as required
  • Perform other related duties as required

  • Attend team meetings and supervision meetings with line manager
  • Assist in the training and mentoring of new staff
